ABSTRACT

Objective: To observe the preventive and therapeutic effect of Huanglian Ejiao Tang on myocardial injury induced by anthracycline chemotherapeutic drugs in all kinds of cancer patients undergoing chemotherapy. Method:We chosen all kinds of cancer patients with combined use of anthracycline chemotherapy drugs in our hospital, 21 days as one cycle. The cardiac toxicity reaction was observed after three continuous chemotherapy cycles. A total of 64 patients who met the dialectical criteria of "imbalance between heart-Yang and kidney-Yin" were randomly divided into treatment group (32 cases) and control group (32 cases). Patients in treatment group were treated with Chinese medicine Huanglian Ejiao Tang based on original chemotherapy regimen, adding and subtracting Chinese medical materials according to the symptoms. Patients in control group continued to maintain the original chemotherapy regimen, and both two groups of patients continued to receive 3 cycles of continuous chemotherapy. By comparing the cardiac function classification and cardiac function tolerance between the 3 cycles and 6 cycles of two groups of patients after chemotherapy; changes of echocardiography index, QTc interval, creatine kinase isoenzyme (CK-MB), Myoglobin (MYO), cardiac troponin I (cTNI)and nitrogenous terminal-pro-brain natriuretic peptide(NT-pro-BNP) concentration value were compared between two groups; and the concentrations of adrenaline (E), norepinephrine (NE) and angiotensin Ⅱ(AngⅡ) were observed and compared; meanwhile, the correlation analysis was carried out at the same time. Result:After 6 cycles of chemotherapy in Chinese medicine treatment group, degree of cardiac function classification and the 6 minute walking heart function tolerance were significantly better than those at the 3 cycles of chemotherapy (PPPPPConclusion:Huanglian Ejiao Tang can reduce the excitability of the symppthetic nervous system (SNS) and renin-angiotensin system(RAS) in human body and inhibit the release level of NE, E and AngⅡ by effect of "invigorating the kidney and clearing the heart". It has a certain preventive and treatment effect on the cardiac toxicity of patients with the cumulative use of anthracycline chemotherapy. To a certain extent, it can inhibit myocardial injury, improve cardiac function and reduce the incidence of cardiovascular events.

ABSTRACT

Background: In the neonatal period, transient tachypnea of the newborn (TTN) is the most frequent cause of early respiratory distress because of delayed resorption of lung fluid, which fills the fetal airways. The inability of the lung to switch from fluid secretion to fluid absorption and immaturity in the expression of epithelial Na+- channels (ENaC) may play an important role in the development of transient tachypnea of the newborn. Aims: Evaluation of inhalation therapy by BETA-2 agonist salbutamol and epinephrine for the management of transient tachypnea of newborn. Methods: The study was a prospective randomized control study. It included 60 neonates diagnosed clinically and radiologicaly as TTN. Inhaled salbutamol, epinephrine or saline 0.45% solution was administered. Results: Comparing studied groups after intervention, highly significant lower respiratory rate, TTN scores, respiratory support and duration of hospital stay was detected in group II (salbutamol group) while no significant difference between groups regarding heart rate after 4 hours from intervention. Conclusion: Inhaled Salbutamol resulted in better outcome in decreasing the respiratory rate, TTN clinical score, lower FIO2, higher O2 saturation, decreasing the duration of respiratory support along with the total duration of hospitalization.

ABSTRACT

OBJECTIVE: To study the protective action on endotheliocytes of fucoidan with different molecular weight from Laminaria japonica, and to explore its mechanism. METHODS: In vivo, endotheliocytes injury model in rats was used to examine the protective action on endotheliocytes of fucoidan with different molecular weight from Laminaria japonica; aortal endothelium section by HE stain under optical microscope was observed, and the the content of von willebrand factor (vWF) in plasma was determined by ELISA. In vitro, endothelial cell culture was used to determine the effect of fucoidan on content of vWF and endothelial microparticles (EMPs). RESULTS: In vivo, aortal endothelium section by HE stain under optical microscope showed low molecular weight (LMW) fucoidan had a remarkable protective effect, and the protection effect of LMWF fucoidans are stronger than middle molecular weight (MMW) fu-coidans. Administration of LMW fucoidan significantly decreased the vWF content. Administration of MMW fucoidan didn't change vWF content. In vitro, Administration of LMW fucoidan significantly decreased the vWF content and EMPs numbers. Administration of MMW fucoidan didn't change the vWF content and EMPs numbers. CONCLUSION: The LMW fucoidan fractions have good protective action on endothelial cells. The protective effect of LMW fucoidans is significantly stronger than MMW fucoidans. The protective action on endotheliocytes of LMW fucoidans relies on decreasing the concentration of EMPs and vWF, and thus inhibiting platelet activation indirectly.

Objective: Because of the received wisdom within our setting that claims that local anesthesia should not be used with adrenalin in hand surgery; we conducted a study using lidocaine with adrenalin, to demonstrate its safety, utility and efficacy. Methods: We conducted a prospective study in which, in wrist, hand and finger surgery performed from July 2012 onwards, we used local anesthesia comprising a 1% lidocaine solution with adrenalin at 1:100,000. We evaluated the quantity of bleeding, systemic alterations, signs of arterial deficit and complications, among other parameters. We described the infiltration techniques for specific procedures individually. Results: We operated on 41 patients and chose to describe separately the raising of a lateral microsurgical flap on the arm, which was done without excessive bleeding and within the usual length of time. In only three cases was there excessive bleeding or use of bipolar tweezers. No systemic alterations were observed by the anesthesiologists or any complications relating to ischemia and necrosis in the wounds or in the fingers, and use of tourniquets was not necessary in any case. Conclusions: Use of lidocaine with adrenalin in hand surgery was shown to be a safe local anesthetic technique, without complications relating to necrosis. It provided efficient exsanguination of the surgical field and made it possible to perform the surgical procedures without using a pneumatic tourniquet, thereby avoiding its risks and benefiting the patient through lower sedation...


Objetivo: Por causa do dogma existente em nosso meio de que não deve ser usada anestesia local com adrenalina na cirurgia da mão, fizemos um estudo com o uso de lidocaína com adrenalina para demonstrar sua segurança, utilidade e eficácia. Métodos: Fizemos um trabalho prospectivo no qual, a partir de julho de 2012, usamos como anestesia local uma solução de lidocaína 1% com adrenalina 1:100.000 nas cirurgias em punho, mão e dedos e avaliamos a quantidade de sangramento, as alteracões sistêmicas, os sinais de déficit arterial e as compliçates, entre outros parâmetros. Descrevemos as técnicas de infiltração de procedimentos específicos individualmente. Resultados: Operamos 41 pacientes e optamos por descrever separadamente um levantamento de retalho microcirúrgico lateral do braço, que ocorreu sem sangramento excessivo e no tempo habitual. Em apenas três casos houve sangramento e uso de pinça bipolar excessivos. Não houve alterações sistêmicas verificadas pelos anestesiologistas ou complicates relacionadas à isquemia e necrose nas feridas ou nos dedos e em nenhum caso foi necessário o uso do torniquete. Conclusões: O uso de lidocaína com adrenalina na cirurgia da mão mostrou-se técnica anestésica local segura, sem compliçates relacionadas à necrose, forneceu campo cirúrgico exsangue eficiente, permitiu os procedimentos cirúrgicos sem uso do torniquete pneumático, evitou seus riscos e beneficiou os pacientes com menor sedação...

ABSTRACT

AIM:To study the effect of Ketangte2 (KTT2)on reducing blood glucose and its mechanism.METHODS:Adrenalin and streptozocin were used to induce hyperglycemia in two mice models,and then treated them with glybenzcyclamide (50 mg/kg),three different dose of KTT2 and normal saline(NS)(0.1 ml/10 g weight)for 15 days.And other healthy mice were set up as control group,During the experiment,the fast blood glucose(FBG),insulin was measured in different time.The pancreas and islets of diabetic mice induced by STZ were studied by pathologic section.RESULTS:In the adrenalin-induced model group,KIT2 could lighten hyperglycemic reaction,which Was significant different from that of model group(P<0.05 or 0.01),while KTT2 high,middle dose group could decrease blood glucose in streptozocin-induced hypoglycemic mice obviously(P<0.01).Histological examination showed that pancreatic island number in pancreas in KTT2 groups increased in comparison with the model group.CONCLUSION:KTT2 can obviously reduce the blood glucose in the two kinds of animal models of hyperglycemia (P<0.05 or 0.01).Its mechanism may be related to the protective effect on islet β cells,thereby increase the insulin secretion.

ABSTRACT

AIM: To study effect of sodium ferulute on experimental arrhythmias. METHODS: Arrhythmias were induces by drugs and myocardial ischemia. RESULTS: Sodium ferulute 0.6 g·kg-1 iv antagonized arrhythmias induced by ouabain, adrenalin and myocardial ischemia (P < 0.01). but showed no effect on aconitine-induced arrhythmia. CONCLUSION: Sodium ferulute has effect on experimental arrhythmias. The mechanism of antiarrhymias is related to the block of potassium channel and β acceptor.

ABSTRACT

Objective To evaluate diagnostic significance of renal arteriography by injecting adrenalin in patients with renal tumour.Methods The renal angiography after administration of adrenalin (6 ?g) in 47 patients with renal malign ant tumour were performed.The angiographic results were analysed in comparison with that of pathology.The diagnostic accuracy parameters including sensitivity,specificity,accuracy,positive prediction rate,negative prediction rate,positive and negative index were caluclated.Results The sensitivity, specificity and accuracy in the detection of renal malign ant tumour were 93% ,94% and 94% respectively. Positive prediction rate were 97%,negative prediction rate was 89%, positive likelihood rate was 15.8, negative likelihood rate was 0.07 and Youden index was 0.87.Conclusion Medicaments angiography by injecting adrenalin is of important diagnostic value for renal tumour,especially for those which are vascularized and could not diagnosed by other technology qualitatively.

ABSTRACT

The effects of Qi Gong health maintenance exercises were compared with a control group of walkers considered to have an equivalent amount of exercise, by measuring the changes in the pulse, systolic blood pressure, diastolic blood pressure and peripheral circulation.<br>Although no significant differences were seen in either the pulse or blood pressure of the control group of walkers when the before and after exercise values were compared, in the Qi Gong group, a decrease in these values was seen over time, with significant decreases from the pre-exercise values indicated in all values 30 minutes after exercise. This is consistent with the findings in a previous report that there was a significant reduction in catecholamine in the Qi Gong group 30 minutes after performing Qi Gong exercises.<br>When the flow rate of the peripheral circulation was measured using Doppler ultrasound equipment, in comparison with the lack of notable change over time seen in the control group, a significant improvement in the peripheral circulation was suggested for the Qi Gong group when the pre- and post- exercise values were compared. It has also been suggested that Qi Gong health maintenance exercises lower stress hormones and act to improve the peripheral blood circulation; a discussion of which is also included in this paper.

ABSTRACT

The method for examining the microcirculation bloodflow of mice mesentery was used to evaluate the action of Shujinhuoxuedingtong wine against microcirculation disturbance caused by adrenaline. The result indicates that the wine can obviously improve the microcirculation function and is responsible for the antistatic action observed. The wine's effect is found to be superior to that of Shujinhuoxuedingtong Powder. The above fingings suggest that the wine have the action of promoting blood circulation to remove blood stasis and improving microcirculation and the effect of promoting the bloodflow perfusion quantity in tissue organs. It's effect is prior to that of powder.
